Healthy Aging Lab Tests: Which Tests Really Matter?

If you want to age well, one of the smartest things you can do is understand what is happening inside your body before symptoms develop. Many conditions that become more common with age, including heart disease, diabetes, kidney disease, and metabolic disorders, can begin quietly. Routine laboratory testing may help identify certain risk factors early, giving you and your healthcare provider an opportunity to discuss preventive care, lifestyle changes, or treatment when appropriate.

At the same time, more testing is not always better. Some laboratory tests are supported by decades of research and routinely used in medical care. Others are helpful only for people with certain symptoms or risk factors. Many of the newer tests marketed for “longevity” or “biological age” are still being studied and may not provide information that changes your medical care.

In this guide, I’ll explain which tests form the foundation of routine health monitoring, when additional biomarkers may be worth discussing, and which newer longevity tests should be approached with caution. I’ll also cover useful measures that do not come from a blood sample, including bone density, body composition, grip strength, and cardiovascular fitness.

No single result can tell you how healthy you are or predict how well you will age. Lab values are most useful when they are interpreted alongside your medical history, medications, symptoms, lifestyle, and changes over time.

Start With the Basics: CBC and CMP

Before considering specialized biomarkers or expensive longevity panels, it makes sense to start with the laboratory tests commonly used to evaluate overall health. Two of the most familiar are the Complete Blood Count and Comprehensive Metabolic Panel.

Complete Blood Count

The Complete Blood Count, commonly called a CBC, measures the cells circulating in your blood. This includes red blood cells, white blood cells, platelets, hemoglobin, hematocrit, and other measurements that provide information about the size and characteristics of your blood cells.

A CBC can help identify possible anemia, infection, immune system problems, certain nutritional deficiencies, and disorders affecting the blood or bone marrow. It may also be used to monitor medical conditions or medications that can affect blood cell counts.

An abnormal result does not automatically mean that something is seriously wrong. Hydration, medications, recent illness, and other factors can influence the numbers. Your healthcare provider will look at the pattern of results and determine whether additional testing is needed.

Comprehensive Metabolic Panel

A Comprehensive Metabolic Panel, or CMP, measures 14 substances in the blood. Together, they provide information about several important areas of health, including:

  • Blood glucose
  • Kidney function
  • Liver function
  • Electrolyte and fluid balance
  • Calcium
  • Protein levels

Some of the familiar markers included in a CMP are glucose, sodium, potassium, calcium, creatinine, blood urea nitrogen, albumin, bilirubin, ALT, AST, and alkaline phosphatase.

Because the CMP covers several body systems at once, it can help uncover changes that may not yet be causing noticeable symptoms. It can also help your healthcare provider monitor an existing condition or check whether a medication is affecting your kidneys, liver, or electrolyte balance.

You may sometimes see a Basic Metabolic Panel, or BMP, instead. A BMP measures eight of the same substances but does not include the liver enzymes, bilirubin, albumin, or total protein found in a CMP.

Kidney and Liver Function

The kidneys and liver perform many essential jobs, from filtering waste and regulating fluid balance to processing nutrients and medications. Several markers included in routine bloodwork can help your healthcare provider evaluate how these organs are functioning.

Kidney Function

Two numbers commonly used to assess kidney function are creatinine and estimated glomerular filtration rate, or eGFR.

Creatinine is a waste product produced during normal muscle metabolism. Healthy kidneys remove it from the blood. When kidney function declines, creatinine may rise because it is not being filtered as efficiently.

Creatinine has limitations, however. Muscle mass, diet, age, certain medications, and other factors can affect the result. A person with more muscle may naturally have a higher creatinine level, while an older adult with very little muscle may have a result that appears normal even when kidney function has declined.

eGFR uses creatinine along with factors such as age and sex to estimate how efficiently the kidneys are filtering blood. One unusual result does not necessarily indicate chronic kidney disease. Healthcare providers often learn more by following eGFR over time and considering it alongside other findings.

Another valuable test is the urine albumin-to-creatinine ratio, or uACR. This test checks for small amounts of albumin leaking into the urine. Albumin in the urine may be an early sign of kidney damage, even when creatinine and eGFR have not yet changed significantly. It is especially important for people with diabetes, high blood pressure, cardiovascular disease, or known kidney disease.

Liver Function

The liver processes nutrients, produces important proteins, helps regulate blood clotting, and breaks down many medications and other substances. Common liver-related markers include ALT, AST, alkaline phosphatase, bilirubin, and albumin.

ALT and AST are enzymes that may rise when liver cells are irritated or injured. ALT is more closely associated with the liver, while AST is also found in muscle and other tissues. This is why an elevated AST result must be interpreted alongside other laboratory findings.

Alkaline phosphatase can provide information about the liver and bile ducts, although it is also found in bone. Bilirubin is produced as red blood cells break down and is normally processed by the liver. Albumin is a protein made by the liver and may help provide information about its ability to perform certain functions.

Abnormal liver results can have many possible causes, including fatty liver disease, alcohol use, medications, supplements, viral infections, bile duct problems, and even strenuous exercise. These tests can identify a possible concern, but they usually cannot determine the cause on their own.

Blood Sugar and Insulin

Changes in blood sugar regulation can develop gradually, often before noticeable symptoms appear. Fasting glucose and hemoglobin A1c are commonly used to screen for prediabetes and diabetes. Fasting insulin may provide additional information in selected situations.

A Snapshot

Fasting Blood Glucose

This test measures your blood sugar after you have not eaten for at least eight hours. It provides a useful snapshot, but it may not reveal blood sugar spikes that occur after meals.

A Longer View

Hemoglobin A1c

The A1c test estimates your average blood sugar over approximately the previous two to three months. It provides a broader view than one fasting measurement and usually does not require fasting.

Additional Insight

Fasting Insulin

This test measures how much insulin your body is producing while fasting. Higher levels may suggest that the body is working harder to keep blood sugar controlled, but fasting insulin is not routinely used to diagnose prediabetes or diabetes.

Insulin resistance can develop when the body's cells stop responding efficiently to insulin. The pancreas may compensate by producing more insulin, sometimes allowing blood glucose to remain within the expected range for a period of time. This is why fasting insulin has attracted attention in preventive and longevity medicine.

However, fasting insulin results can vary, and there is no single cutoff used to diagnose insulin resistance in every patient. Healthcare providers generally rely on established glucose tests, medical history, and individual risk factors when deciding whether further evaluation is appropriate.

Cholesterol and Cardiovascular Risk

Most people are familiar with total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides. These measurements remain important, but they do not always tell the whole story.

Some people develop cardiovascular disease even though their standard cholesterol results do not appear especially concerning. Additional markers may help provide a clearer picture of risk, particularly when someone has diabetes, high triglycerides, metabolic syndrome, kidney disease, or a family history of premature heart disease.

Apolipoprotein B

Apolipoprotein B, usually called ApoB, provides an estimate of the number of particles in the blood that can contribute to plaque buildup in the arteries.

Each potentially artery-clogging particle, including LDL, VLDL, IDL, and lipoprotein(a), carries one ApoB protein. A standard LDL result measures the amount of cholesterol being carried inside LDL particles. ApoB helps estimate how many of these potentially harmful particles are circulating.

This distinction can matter because two people with similar LDL cholesterol levels may have different numbers of particles. ApoB testing may be especially helpful for people with high triglycerides, diabetes, obesity, metabolic syndrome, or results that do not seem to provide a consistent picture of cardiovascular risk.

Lipoprotein(a)

Lipoprotein(a), abbreviated Lp(a), is a cholesterol-carrying particle that is largely determined by genetics. An elevated level is associated with a higher risk of heart attack, stroke, peripheral artery disease, and aortic valve stenosis.

Unlike many other cardiovascular risk factors, Lp(a) is not significantly lowered through diet and exercise alone. Because the level usually remains relatively stable throughout life, current cardiovascular guidelines recommend that every adult have Lp(a) measured at least once.

Testing is particularly important when there is a personal or family history of premature cardiovascular disease or unexpectedly high cholesterol. Knowing that Lp(a) is elevated may influence how aggressively your healthcare provider addresses other risk factors, including LDL cholesterol, blood pressure, smoking, and blood sugar.

High-Sensitivity C-Reactive Protein

High-sensitivity C-reactive protein, or hs-CRP, measures low levels of inflammation in the body. It may provide additional information about cardiovascular risk, particularly when someone’s risk is otherwise uncertain.

However, hs-CRP is not specific to heart disease. Levels can temporarily rise because of an infection, dental problem, arthritis flare, recent injury, or surgery. An elevated result may need to be repeated when you are feeling well before it can be meaningfully interpreted.

These additional tests do not replace the standard lipid panel. They add context and may help your healthcare provider develop a more complete assessment of your cardiovascular risk.

Thyroid Function

The thyroid helps regulate metabolism, energy production, body temperature, heart rate, digestion, and many other functions. Thyroid problems become more common with age, and symptoms can be easy to mistake for normal aging.

An underactive or overactive thyroid may contribute to fatigue, weight changes, muscle weakness, mood changes, memory difficulties, changes in heart rate, high cholesterol, or constipation. However, these symptoms can have many possible causes, so blood testing is needed to determine whether the thyroid may be involved.

TSH

Thyroid-stimulating hormone, or TSH, is usually the first test used to evaluate thyroid function. TSH is produced by the pituitary gland and signals the thyroid to make thyroid hormone.

In most cases, a high TSH may suggest an underactive thyroid, while a low TSH may suggest an overactive thyroid. The result must still be interpreted alongside symptoms, medications, medical history, and other thyroid tests.

Free T4 and Free T3

Free T4 measures the main hormone produced by the thyroid gland. It is often checked when the TSH result is abnormal or when a healthcare provider needs additional information.

Free T3 testing is more useful in certain situations, particularly when an overactive thyroid is suspected. It is not routinely needed for every thyroid evaluation and is generally less helpful when evaluating an underactive thyroid.

Pharmacist’s Tip: Biotin, an ingredient commonly found in hair, skin, and nail supplements, can interfere with some thyroid laboratory tests and make the results appear abnormal. Tell your healthcare provider about any biotin-containing supplements before testing and ask whether they should be temporarily stopped.

Vitamins, Minerals, and Iron Stores

Nutrient testing can be useful when a deficiency is suspected or when age, diet, medications, or a medical condition increases the risk. These tests are not automatically necessary for everyone.

Vitamin D

Vitamin D supports bone strength, muscle function, and normal immune function. Deficiency is more common among older adults, people with limited sun exposure, and those with certain conditions that affect nutrient absorption.

The usual blood test measures 25-hydroxyvitamin D. Routine testing is not recommended for every healthy adult, but it may be appropriate when deficiency or a bone-related condition is suspected. Testing can also help guide supplementation because excessive vitamin D can be harmful.

Vitamin B12

Vitamin B12 is needed for healthy nerves, red blood cell formation, DNA production, and brain function. Deficiency may cause fatigue, numbness, tingling, balance problems, or memory changes.

The risk of deficiency can increase with age because the body may not absorb B12 as efficiently. Long-term use of metformin or certain acid-reducing medications may also affect B12 levels.

Ferritin

Ferritin reflects the amount of iron stored in the body. A low ferritin level may reveal iron deficiency before anemia becomes apparent on a CBC.

A high ferritin result does not always mean that someone has too much iron. Ferritin may also rise with inflammation, infection, liver disease, or certain metabolic conditions. It is usually interpreted alongside other iron studies and the CBC.

Magnesium

Magnesium supports muscle and nerve function, heart rhythm, blood pressure regulation, bone health, and blood sugar control.

A standard serum magnesium test measures only the small amount circulating in the blood. The body tightly regulates this level, so a normal result may not always reflect total magnesium stores. Your healthcare provider will also consider symptoms, diet, medications, and kidney function.

Homocysteine

Homocysteine is an amino acid produced during normal metabolism. Elevated levels may occur when the body does not have enough vitamin B12, folate, or vitamin B6. Kidney disease, certain medications, genetics, and other health conditions can also affect the result.

Higher homocysteine levels have been associated with cardiovascular disease, stroke, and cognitive decline. However, lowering homocysteine with B vitamin supplements has not consistently been shown to prevent heart attacks or other cardiovascular events.

For that reason, homocysteine is not generally used as a routine screening test for every healthy adult. It may provide useful information in selected situations, particularly when a vitamin B12 or folate deficiency is suspected or when a healthcare provider is investigating an unusually elevated result.

Pharmacist's Tip: Do not assume that more is better when a vitamin or mineral level is low. The appropriate supplement and amount depend on the cause of the deficiency, your medications, kidney function, and other health factors.

Hormone Testing: Is It Right for Everyone?

Hormones influence muscle mass, bone health, metabolism, mood, sexual health, and energy. This has made hormone panels popular in longevity medicine, but testing every hormone simply because someone is getting older is rarely helpful.

Hormone testing is usually most valuable when a person has symptoms, a medical condition, or a treatment that gives the healthcare provider a specific reason to investigate.

Testosterone

In men, testosterone levels often decline gradually with age. Low testosterone may contribute to reduced muscle mass, decreased strength, low libido, fatigue, mood changes, or reduced bone density. However, these symptoms can also have many other causes.

A diagnosis should not be based on symptoms or one laboratory result alone. Testosterone is generally measured in the early morning, when levels tend to be highest. If the result is low, the test is usually repeated on a different morning before a diagnosis or treatment decision is made.

Testosterone treatment is not an anti-aging therapy for every older man. The potential benefits and risks should be evaluated individually.

Estrogen

Estrogen levels change considerably during perimenopause and menopause. For most women experiencing menopause at the expected age, blood testing is not routinely needed to confirm what is happening.

Estrogen and other hormone tests may be appropriate when symptoms are unusual, menstrual changes occur earlier than expected, the diagnosis is uncertain, or another hormonal condition is suspected.

Hormone results can fluctuate and may be affected by medications, supplements, illness, and the timing of the test. The number is only useful when interpreted in the context of the person being tested.

Healthy Aging Beyond Blood Tests

Blood tests can provide valuable information, but they cannot show how strong your bones are or how much of your body weight comes from muscle rather than fat. These measurements become increasingly important as we age because they are closely connected to mobility, independence, and fracture risk.

Bone Density

Osteoporosis can develop silently for years. For many people, the first noticeable sign is a fracture.

A Dual-Energy X-ray Absorptiometry scan, usually called a DEXA or DXA scan, measures bone mineral density and is the standard test used to diagnose osteoporosis. The scan is painless, takes only a few minutes, and exposes you to a very small amount of radiation.

Current recommendations support bone density screening for women age 65 and older. Screening may begin earlier for postmenopausal women who have an increased risk of osteoporosis. A healthcare provider may also recommend testing based on factors such as a previous fracture, low body weight, long-term corticosteroid use, family history, or certain medical conditions.

Screening recommendations for men are less uniform, so the decision is generally based on age and individual risk factors.

Body Composition

A bathroom scale tells you how much you weigh, but it cannot tell you what that weight consists of. Two people with the same weight may have very different amounts of muscle, body fat, visceral fat, and bone mass.

This distinction matters because losing muscle while gaining fat may not cause a dramatic change on the scale. It can still affect strength, balance, blood sugar regulation, and the ability to remain active.

Body composition may be estimated using DEXA scans, bioelectrical impedance devices, air displacement testing, or skinfold measurements. Each method has limitations, and results from different devices are not always directly comparable.

When tracking body composition, it is often more helpful to use the same method under similar conditions and watch the trend over time.

Healthy aging lab tests vs. functional tests

Protein: The Building Blocks for Muscle

If resistance training is the engine that drives muscle growth, protein is the raw fuel. Getting enough high-quality protein in your diet becomes exponentially more important as the birthdays add up.

The current standard Recommended Dietary Allowance (RDA) for protein is about 0.36 grams per pound of body weight. However, the RDA is designed to prevent deficiency in the average adult, not to optimize muscle health or support healthy aging. As we get older, our muscles become less responsive to protein, a phenomenon known as anabolic resistance. To help maintain muscle mass and strength, many experts recommend that older adults consume approximately 0.45 to 0.55 grams of protein per pound of body weight each day, with even higher amounts during illness or recovery from injury.

Individuals who are actively trying to build muscle through resistance training often benefit from 0.7 to 1 gram of protein per pound or more of body weight daily, depending on their training goals, overall health and kidney function.

Where to Find High-Quality Fuel

To maximize muscle repair, focus on protein sources rich in essential amino acids:

  • Animal Sources: Lean beef, poultry (chicken, turkey), wild-caught fish, eggs, and dairy products like Greek yogurt or cottage cheese.
  • Plant-Based Sources: Soy (tofu, tempeh), lentils, chickpeas, black beans, and edamame.

Time Your Meals for Maximum Growth

How you eat your protein matters just as much as how much you eat. Most people follow a steep pyramid: a tiny bit of protein at breakfast, a moderate amount at lunch, and a massive portion at dinner.

The Pro Tip: To keep your muscle-building machinery running all day long, distribute your protein evenly across breakfast, lunch, and dinner. Aiming for roughly 25 to 30 grams of protein per meal triggers muscle synthesis far more effectively than loading up entirely at the end of the day.

Protein & Healthy Aging

Younger Adults vs. Older Adults

As we age, our muscles become less responsive to dietary protein, a phenomenon known as anabolic resistance. As a result, many experts recommend that older adults consume more protein than the minimum Recommended Dietary Allowance (RDA).

Medications like semaglutide, tirzepatide, and retatrutide have completely revolutionized obesity treatment. While their ability to drive weight loss is undeniable, a significant portion of that dropped weight often comes from lean skeletal muscle rather than fat.
